Food Vendor Forms

*NOTE: Approved food vendors are required to complete the following forms. Please follow the return instructions provided.

FIRST: If you are selling a consumable of any kind, you must first complete a Mobile Retail Food Application and return to Atlantic County Division of Public Health.

Complete this form first and return to the Atlantic County Division of Public Health.

If you have prior approval from the Atlantic County Division of Public Health, please complete and return to them.

MOBILE RETAIL FOOD APPLICATION AMENDMENT: This application is to be used by vendors who have received full prior approval from a Health Department and plan on participating in a special event.

CERTIFICATE OF LIABILITY INSURANCE: Please return to Cape Atlantic Irish Association.

GALLOWAY TOWNSHIP FIRE PERMIT APPLICATION: Food vendors using open burning/cooking methods must have a Fire Permit. The fire permit is $54 and is good for the whole year.

The **primary logo** for the **2025 Smithville Irish Festival** features a circular Celtic knot border in deep green, symbolizing Irish heritage. Inside, bold white lettering spells **"Smithville"**, with a golden harp integrated into the "V," representing Irish culture. The festival’s name, **Cape Atlantic Irish Association**, and location, **Historic Smithville, NJ**, are elegantly arranged within the design, along with a small shamrock accent.
